We have a full-time vacancy for a Practice Manager for the Out of County Hospital Discharge team, which is based in Lewes. This team supports adults to be discharged from several acute hospitals and intermediate care units locally, including the Royal Sussex County Hospital, Princess Royal Hospital, Tunbridge Wells and Maidstone Hospitals. They also undertake review work and have a significant role in safeguarding adults.

As a Practice Manager, you will play a key role in the day-to-day management of the team. The team undertake Care Act assessment of needs and reviews of adults and carers who have been referred to your team and who are ready for discharge from hospital. You will be expected to work in a multidisciplinary way to support a person's discharge and contribute to discussions that promote the person's independence and best interest principles.

As the manager of the team you will be responsible for oversight of the safeguarding enquiries held by the team, ensuring that we fulfil our duty under s42.2 of the Care Act 2014 by working with partner agencies to assess, and minimise safeguarding risk to residents of East Sussex.

We are looking for candidates who:

We have excellent relationships with local partners and hospital trusts, and you will have the opportunity to work alongside other professionals to ensure good outcomes for the individual's we are working with. Where necessary, we work with our local Integrated Commissioning Boards (ICB's) to ensure people we serve get access to continuing care funding.
