WANTED "Someone Like You"

5 x Permanent Grade.5 Community Reablement Workers in the north areas of Cheshire East - Knutsford/Wilmslow/Poynton/Macclesfield and rural. The office is based at Macclesfield Town Hall.

These are permanent roles with flexible contracts available between 15 and 37 hours per week. The service operates primarily between 8:00am and 10:00pm, contracts are from 7:00am to 11:00pm to support emergencies. Travel time is included within your shift. Due to the nature of the role, a full UK driving licence and access to a vehicle are essential.

ABOUT THE ROLE

Reablement is an integrated service with health and provides a short-term intervention to encourage individuals to achieve maximum independence with personal care and daily living skills, working in a person-centred manner. Our remit is to support discharges from the hospitals, or work as part of the Prevent, Reduce, Enable Team with people at home, to complete assessments to enable individuals to regain their independence or assess for ongoing care needs and rehabilitation of therapy exercises, and low-level health tasks.

Experience is welcome but it is not required as full induction will be given along with all necessary training and support however if you are caring, friendly and have a commitment to enrich people's lives whilst treating individuals with dignity and respect, we would welcome your application.

We're looking for someone like you to join our dedicated team at Community Reablement Services where we support individuals being discharged from hospital or requiring an assessment at home to regain their independence to lead fulfilling lives and stay connected to their community or assess for long-term care needs.

🏡 About the Role

You'll be part of a team providing person-centred support, helping people regain their independence or assess for long-term care needs, provide therapy and low-level health tasks, Support includes:

  • Personal care
  • Medication administration
  • Support to regain daily living skills
  • Assessment of care needs
  • Therapy exercises
  • Low-level health tasks
  • Community engagement

We work closely with individuals, families, and professionals to ensure services reflect each person's choices, aspirations, and preferences.

What We're Looking For

No experience? No problem!
We provide a full induction, ongoing training, and support. What matters most is that you:
  • Are compassionate and respectful
  • Confident decision-maker with a proactive approach to problem-solving
  • Effective teamwork and excellent communication skills
  • An enthusiastic, flexible, and reliable approach to work
  • Have excellent spoken English
  • Work to the Council`s values and behaviours
  • Hold a full UK driving licence
  • Can work a rota including alternate weekends and bank holidays

💷 Pay & Benefits
  • £13.69 /hr, rising to £14.59/hr
  • Time and a half on weekends and bank holidays
